New Guidelines Urge OTC Painkillers, Not Opioids\" class=\"healthday_image\" src=\"https:\/\/media.healthday.com\/Images\/icimages\/head.jpg\"\/><b>By Amy Norton<\/b><br \/><i>HealthDay Reporter<\/i><\/p>\n<p>MONDAY, Aug. 17, 2020 (HealthDay News)<\/p>\n<p>People with common muscle and joint injuries should skip opioids and instead reach for <a href=\"https:\/\/www.medicinenet.com\/over-the-counter_products\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">over-the-counter<\/a> <a href=\"https:\/\/www.medicinenet.com\/pain_management\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">pain<\/a> relievers, new treatment guidelines suggest.<\/p>\n<p>The recommendations, from the American College of Physicians and American Academy of Family Medicine (AAFP), cover acute musculoskeletal injuries &#8212; woes ranging from sprained joints and strained muscles, to inflamed tendons and <a href=\"https:\/\/www.medicinenet.com\/whiplash\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">whiplash<\/a>.<\/p>\n<p>The groups say that in general, treatment should start on the conservative end, with <a href=\"https:\/\/www.medicinenet.com\/pain_quiz\/quiz.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"quiz\">pain<\/a>-relieving creams and gels.<\/p>\n<p>If that&#8217;s not enough, common oral painkillers are good options. They include <a href=\"https:\/\/www.medicinenet.com\/acetaminophen\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">acetaminophen<\/a> and nonsteroidal anti-inflammatory <a href=\"https:\/\/www.medicinenet.com\/drugs_what_you_should_know_about_your_drugs\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">drugs<\/a> (<a href=\"https:\/\/www.medicinenet.com\/nonsteroidal_antiinflammatory_drugs\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">NSAIDs<\/a>), such as <a href=\"https:\/\/www.medicinenet.com\/ibuprofen\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">ibuprofen<\/a> and <a href=\"https:\/\/www.medicinenet.com\/naproxen\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">naproxen<\/a>.<\/p>\n<p>On the other hand, the guidelines discourage prescribing opioids such as <a href=\"https:\/\/www.medicinenet.com\/oxycodone\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">OxyContin<\/a>, <a href=\"https:\/\/www.medicinenet.com\/hydrocodoneacetaminophen\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">Vicodin<\/a> and <a href=\"https:\/\/www.medicinenet.com\/oxycodone_and_acetaminophen\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">Percocet<\/a>. While the <a href=\"https:\/\/www.medicinenet.com\/drugs_what_you_should_know_about_your_drugs\/article.htm\" rel=\"pharm\" onclick=\"wmdTrack('embd-lnk');\">drugs<\/a> can help with shorter-term <a href=\"https:\/\/www.medicinenet.com\/pain_management\/article.htm\" rel=\"dt\" onclick=\"wmdTrack('embd-lnk');\">pain<\/a>, they are also potentially addictive and carry a risk of serious side effects.<\/p>\n<p>&#8220;Opioids should not be routinely used for acute <a href=\"https:\/\/www.medicinenet.com\/pain_quiz\/quiz.htm\" rel=\"quiz\" onclick=\"wmdTrack('embd-lnk');\">pain<\/a>,&#8221; said Dr. Timothy Wilt, who chaired the ACP Clinical Guidelines committee.<\/p>\n<p>&#8220;Acute&#8221; means pain that has been around for less than a month. Other research has found that opioids are also of little use for most cases of <a href=\"https:\/\/www.medicinenet.com\/chronic_pain\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">chronic pain<\/a> unrelated to <a href=\"https:\/\/www.medicinenet.com\/cancer\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">cancer<\/a>.<\/p>\n<p>That&#8217;s not to say no one who is in acute pain should get an opioid prescription. Some people may need the medications for a short time, said Wilt, a professor of medicine at the Minneapolis VA Center for Care Delivery and Outcomes Research.<\/p>\n<p>But in general, he said, the evidence shows &#8220;other options are safer and more effective.&#8221;<\/p>\n<p>The guidelines, published Aug. 17 in the <i>Annals of Internal Medicine<\/i>, are based on a review of over 200 <a href=\"https:\/\/www.medicinenet.com\/clinical_trials\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"proc\">clinical trials<\/a> testing treatments for recent musculoskeletal injuries. Patients had a range of injuries &#8212; from sprained ankles to torn hamstrings to <a href=\"https:\/\/www.medicinenet.com\/whiplash\/article.htm\" rel=\"dt\" onclick=\"wmdTrack('embd-lnk');\">whiplash<\/a> &#8212; but did not have <a href=\"https:\/\/www.medicinenet.com\/low_back_pain\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">low back pain<\/a>.<\/p>\n<p>A set of 2017 guidelines tackled <a href=\"https:\/\/www.medicinenet.com\/low_back_pain_pictures_slideshow\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"slide\">low back pain<\/a>, and came to similar conclusions: If medications are used, <a href=\"https:\/\/www.medicinenet.com\/nonsteroidal_antiinflammatory_drugs\/article.htm\" rel=\"pharm\" onclick=\"wmdTrack('embd-lnk');\">NSAIDs<\/a> should be the first choice, said Dr. Gary LeRoy, president of the AAFP.<\/p>\n<p>The new guidelines suggest topical versions of NSAIDs &#8212; with or without menthol gel &#8212; be tried first.<\/p>\n<p>That emphasis on topical painkillers &#8212; and stance on opioids &#8212; are good to see, according to Dr. Houman Danesh, a <a href=\"https:\/\/www.medicinenet.com\/pain_management\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">pain management<\/a> specialist who was not involved in the guidelines.<\/p>\n<p>&#8220;It&#8217;s important for doctors to feel supported in <i>not<\/i> using opioids,&#8221; said Danesh, who directs the division of integrative <a href=\"https:\/\/www.medicinenet.com\/pain_management\/article.htm\" rel=\"dt\" onclick=\"wmdTrack('embd-lnk');\">pain management<\/a> at Mount Sinai Hospital in New York City.<\/p>\n<p>With the United States years into an opioid abuse epidemic, medical societies have been advising doctors to rein in their prescribing of the drugs when other options are available.<\/p>\n<p>Yet the drugs are still commonly prescribed for <a href=\"https:\/\/www.medicinenet.com\/pain_management_musculoskeletal_pain\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">musculoskeletal pain<\/a>, Wilt said. Danesh agreed, noting that patients sometimes ask for them.<\/p>\n<p>It is true that NSAIDs can have side effects, like stomach upset or <a href=\"https:\/\/www.medicinenet.com\/internal_bleeding\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">internal bleeding<\/a> &#8212; especially if used for a prolonged time. And some people are at increased risk of side effects from NSAIDs or <a href=\"https:\/\/www.medicinenet.com\/acetaminophen\/article.htm\" rel=\"pharm\" onclick=\"wmdTrack('embd-lnk');\">acetaminophen<\/a>, including older adults and people with <a href=\"https:\/\/www.medicinenet.com\/heart_how_the_heart_works\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">heart<\/a>, kidney or <a href=\"https:\/\/www.medicinenet.com\/liver_disease\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">liver disease<\/a>.<\/p>\n<p>That&#8217;s why topical NSAIDs are suggested as a first choice: They have fewer side effects, LeRoy said.<\/p>\n<p>On balance, though, NSAIDs and acetaminophen are safer than opioids, and often ease acute pain, the guidelines say.<\/p>\n<p>Danesh did note that inflammation is part of the body&#8217;s natural response to acute injury. Trials show they help ease pain and &#8212; in the case of acupressure &#8212; may improve people&#8217;s physical functioning.<\/p>\n<p>Some others &#8212; like physical therapy and <a href=\"https:\/\/www.medicinenet.com\/massage_therapy\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"sub\">massage<\/a> &#8212; were not cited in the guidelines but might help some people, according to LeRoy.<\/p>\n<p>The good news is <a href=\"https:\/\/www.medicinenet.com\/pain_management_musculoskeletal_pain\/article.htm\" rel=\"dt\" onclick=\"wmdTrack('embd-lnk');\">musculoskeletal pain<\/a> usually wanes within four to six weeks, according to Danesh. &#8220;If it doesn&#8217;t,&#8221; he said, &#8220;you may need a referral to someone like me.&#8221;<\/p>\n<p>Ultimately, Danesh said, it&#8217;s best to try to figure out the root cause of musculoskeletal pain.
