{"id":26012,"date":"2020-10-15T09:44:05","date_gmt":"2020-10-15T09:44:05","guid":{"rendered":"https:\/\/bengalnewstimes.com\/?p=26012"},"modified":"2020-10-15T09:44:05","modified_gmt":"2020-10-15T09:44:05","slug":"new-procedure-may-end-insulin-for-type-2-diabetes","status":"publish","type":"post","link":"https:\/\/bengalnewstimes.com\/?p=26012","title":{"rendered":"New Procedure May End Insulin for Type 2 Diabetes"},"content":{"rendered":"<p><\/p>\n<div>\n<p>                            <img decoding=\"async\" alt=\"News Picture: Minimally Invasive Procedure May Free Type 2 Diabetics From Insulin\" class=\"healthday_image\" src=\"https:\/\/media.healthday.com\/Images\/icimages\/insulin109.jpg\"\/><b>By Serena Gordon<\/b><br \/><i>HealthDay Reporter<\/i><\/p>\n<p>WEDNESDAY, Oct. 14, 2020<\/p>\n<p>A small study suggests that a new procedure that treats part of the intestine just beyond the stomach may allow people with <a href=\"https:\/\/www.medicinenet.com\/type_2_diabetes\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">type 2 diabetes<\/a> to safely stop taking <a href=\"https:\/\/www.medicinenet.com\/insulin\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">insulin<\/a>.<\/p>\n<p>The procedure &#8212; which resurfaces the duodenum &#8212; was combined with a popular kind of <a href=\"https:\/\/www.medicinenet.com\/diabetes_mellitus\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">diabetes<\/a> medication called GLP-1 receptor agonists (such as <a href=\"https:\/\/www.medicinenet.com\/liraglutide\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">Victoza<\/a>, <a href=\"https:\/\/www.medicinenet.com\/trulicity_dulaglutide\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"pharm\">Trulicity<\/a>, Ozempic) and counseling on lifestyle factors, such as <a href=\"https:\/\/www.medicinenet.com\/nutrition\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"sub\">nutrition<\/a> and physical activity.<\/p>\n<p>Six months after treatments began, three-quarters of participants taking <a href=\"https:\/\/www.medicinenet.com\/insulin\/article.htm\" rel=\"pharm\" onclick=\"wmdTrack('embd-lnk');\">insulin<\/a> no longer needed it. The amount of fat stored in their livers dropped from 8% to less than 5%.<\/p>\n<p>&#8220;The duodenum harbors a broad potential for the treatment of <a href=\"https:\/\/www.medicinenet.com\/type_2_diabetes_pictures_slideshow\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"slide\">type 2 diabetes<\/a> and this combination treatment could be a game-changing approach in the treatment of <a href=\"https:\/\/www.medicinenet.com\/type_2_diabetes\/quiz.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"quiz\">type 2 diabetes<\/a> and the <a href=\"https:\/\/www.medicinenet.com\/metabolic_syndrome\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">metabolic syndrome<\/a>,&#8221; said lead researcher Dr. Suzanne Meiring, of Amsterdam University Medical Center in the Netherlands.<\/p>\n<p>This preliminary study included 16 patients, all of whom underwent Duodenal Mucosal Resurfacing (DMR). There was no placebo group or medication-only group for comparison. The study was funded by Fractyl Laboratories, which developed the procedure.<\/p>\n<p>DMR is a minimally invasive procedure that relies on an endoscope &#8212; a narrow, flexible tube containing a light and video camera that lets your doctor see inside the body. The endoscope may also carry special tools for treatment. The tube is threaded down the throat into the <a href=\"https:\/\/www.medicinenet.com\/the_digestion_process_organs_and_functions\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">digestive system<\/a>.<\/p>\n<p>In DMR, the endoscope is guided to the duodenum, where doctors then resurface, or ablate, its lining. Meiring said it&#8217;s not yet clear why the procedure works.<\/p>\n<p>&#8220;We think the effects result from a combination of changes that occur when the duodenal mucosa is ablated and rejuvenated,&#8221; she said. &#8220;We think that changes in hormonal signaling, including the gut hormone GLP-1, bile acid compositions and the microbiome play an important role.&#8221;<\/p>\n<p>The 16 participants had type 2 <a href=\"https:\/\/www.medicinenet.com\/type_2_diabetes_pictures_slideshow\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"slide\">diabetes<\/a> for an average 11 years. On average, they had been on insulin just under three years. None had taken a GLP-1 receptor agonist before the study.<\/p>\n<p>At the outset, their average A1C levels &#8212; an estimate of blood <a href=\"https:\/\/www.medicinenet.com\/sugar_addiction_pictures_slideshow\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"slide\">sugar<\/a> levels over two to three months &#8212; were under 8%. After 12 months, the average fell to 6.7%. For most adults, the American <a href=\"https:\/\/www.medicinenet.com\/diabetes_quiz\/quiz.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"quiz\">Diabetes<\/a> Association recommends aiming for an A1C below 7%.<\/p>\n<p>After the DMR procedure, patients were given a specific <a href=\"https:\/\/www.medicinenet.com\/diet_plans_and_programs\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"sub\">diet<\/a> for two weeks. After that, they began taking the GLP-1 receptor agonist medication.<\/p>\n<p>Meiring said researchers added the drug because it also targets the duodenum. They hoped it would boost the effects of DMR, &#8220;possibly even causing a synergistic effect,&#8221; she said.<\/p>\n<p>She said she doesn&#8217;t think the positive effects in this study stem from just the drug treatment. Other studies have shown that about 10% of people who start GLP-1 therapy are able to get off insulin, Meiring said. In this study, 75% of those taking insulin were able to stop.<\/p>\n<p>Participants who weren&#8217;t insulin-free after 12 months needed only about half the insulin they required before the procedure.<\/p>\n<p>Their average body mass index (<a href=\"https:\/\/www.medicinenet.com\/body_mass_index_bmi\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"sub\">BMI<\/a>) &#8212; a measure of body fat based on height and weight &#8212; dropped from just under 30 to a near-normal 25.5 after 12 months.<\/p>\n<p>The researchers said there were no serious procedure-related side effects.<\/p>\n<p>Dr. Akankasha Goyal, an endocrinologist at NYU Langone Health in New York City, reviewed the findings.<\/p>\n<p>&#8220;This is a simple, single procedure that seems to have amazingly positive results,&#8221; she said. &#8220;But, this is a small group from a homogenous population. It looks hopeful, but we need to see the larger trials. And, we need to know: how long does the procedure last?&#8221;<\/p>\n<p>Dr. Mitchell Roslin, chief of <a href=\"https:\/\/www.medicinenet.com\/obesity_weight_loss\/article.htm\" onclick=\"wmdTrack('embd-lnk');\" rel=\"dt\">obesity<\/a> surgery at Lenox Hill Hospital in New York City, called findings &#8220;very exciting&#8221; but urged caution.<\/p>\n<p>Roslin said the procedure may not have a direct effect on the <a href=\"https:\/\/www.medicinenet.com\/diabetes_mellitus\/article.htm\" rel=\"dt\" onclick=\"wmdTrack('embd-lnk');\">diabetes<\/a>. I would bet it&#8217;s an association.&#8221;<\/p>\n<p>The findings were presented Tuesday at an online meeting of United European Gastroenterology. Findings presented at meetings are typically viewed as preliminary.<\/p>\n<p>Meiring said that placebo-controlled <a href=\"https:\/\/www.medicinenet.com\/clinical_trials\/article.htm\" rel=\"proc\" onclick=\"wmdTrack('embd-lnk');\">clinical trials<\/a> of DMR have started or will begin soon at 35 hospitals, including some in the United States.<\/p>\n<p class=\"credits\"><img decoding=\"async\" alt=\"MedicalNews\" border=\"0\" src=\"https:\/\/images.medicinenet.com\/images\/HealthDay\/healthday.png\" style=\"border: none;\"\/><br \/>Copyright \u00a9 2020 HealthDay.
